and Permission for construction and installation of this system is granted in accordance to Title 10, Chapter 10, Subchapter 10A of North Carolina Administrative Code, Section .1948 (c) of Laws and Rules for Sanitary Sewage Collection, Treatment Disposal. The conditions and provisions of this section are considered to have been satisfied and acceptance of the attached plans is based upon the "Approval With Provisos by Department of Human Resources Division of Health Services Environmental Health Section" dated August 3, 1988.

Issuance of this Operations Permit will be contingent upon the following additional requirements: Environmental 1) the residential structure location and dimensions shall be as specifically indicated on the attachment labelled Exhibit A; 2) 3) 4) 5) inspections of system construction and installation and site modifications shall be completed and approved in accordance to the requirements of the attachment labelled Exhibit C; the design engineer shall certify in writing to the local Environmental Health Division that the system has been constructed and installed as specified on the plans labelled Exhibit B;
